Abstract: Transitional support service provides a stop-gap, timely and enhanced transitional support to persons in mental recovery to be discharged from hospital and waitlisted for halfway house service. Through medical-social collaboration, the clustered-based multi-disciplinary outreach support teams will help the service users to smoothly transition into halfway house service. Also, by strengthening support to their families/carers, service users' individual needs amid their recovery pathway will be met and they may reintegrate into the community.

摘要: 「精神復元人士過渡支援服務」為正輪候中途宿舍服務的離院人士提供持續、適時且加強的過渡支援服務，透過地區為本的多專業支援外展隊，以醫社協作模式，協助服務使用者順利入住中途宿舍適應生活 ；並透過加強家庭/照顧者支援，令服務使用者在復元路上所面對的個別需要得到滿足，融入社區適應生活。
