Abstract: The Social Welfare Department has sought from the Lotteries Fund to implement “The Pilot Project on Integrated Community Rehabilitation Centre” (the Pilot Project) through 2 ICRCs to provide a continuum of coherent and interconnected community support services for persons with disabilities having intensive care needs and living at home and to provide support to their carers. The ICRCs set up under the Pilot Project will provide integrated day care and home-based support services to be delivered in a flexible manner which will cater for the actual impairment level and changing needs of the persons with severe disabilities.

摘要: 社會福利署獲獎券基金撥款推行綜合社區康復中心試驗計劃(「試驗計劃」)，透過兩間綜合社區康復中心為有高度照顧需要並在家中生活的嚴重殘疾人士提供連貫連續、互相銜接的社區支援服務，並為他們的照顧者提供支援。試驗計劃下設立的綜合社區康復中心將會靈活地融合日間護理中心及家居照顧服務，按嚴重殘疾人士的實際缺損程度及服務需要的轉變提供服務。

摘要: 社会福利署获奖券基金拨款推行综合社区康复中心试验计划(「试验计划」)，透过两间综合社区康复中心为有高度照顾需要并在家中生活的严重残疾人士提供连贯连续、互相衔接的社区支援服务，并为他们的照顾者提供支援。试验计划下设立的综合社区康复中心将会灵活地融合日间护理中心及家居照顾服务，按严重残疾人士的实际缺损程度及服务需要的转变提供服务。
