Abstract: Home Care Services Teams provide "Home Care Services for Frail Elderly Persons", which are home-based community care services providing a range of care and support services to frail elderly persons aged 60 or above living in the community and who are assessed and recommended for Community Care Services or Residential Care Services under the Standardised Care Need Assessment Mechanism for Elderly Services. The services aim at facilitating service users to continue ageing in place for as long as possible and maintaining their optimal level of functioning, while providing various kinds of support and assistance to carers.

摘要: 由家居照顧服務隊提供的「體弱長者家居照顧服務」，是以家居為本的長者社區照顧服務，為區內60歲或以上，經「安老服務統一評估機制」評估後建議適合接受社區照顧服務或院舍照顧服務的體弱長者，提供一系列的照顧及支援服務，協助他們盡可能繼續居家安老，保持最佳活動能力；同時為照顧者提供各類支援和協助。
